About the present. 09:00.751 --> 09:04.342 Recently, the emergency room team dealt with nine patients 09:04.342 --> 09:08.342 who came in early morning between 0500 and 0600. 09:09.247 --> 09:12.372 Two groups came in associated with motor vehicle accidents 09:12.372 --> 09:16.243 including a group who crashed in a ditch while on patrol. 09:16.243 --> 09:18.994 We also had a patient come in with a myocardial infarction 09:18.994 --> 09:22.568 or heart attack, and other patients requiring attention 09:22.568 --> 09:24.651 due to illness or injury. 09:24.705 --> 09:27.105 All the beds in the emergency room were filled 09:27.105 --> 09:29.401 and our team needed to use some of the exam rooms 09:29.401 --> 09:32.863 here in our primary care clinic as emergency spaces. 09:32.863 --> 09:37.030 The team handled things superbly for all patients. 09:37.089 --> 09:39.144 And the heart attack patient went out on 09:39.144 --> 09:40.668 an emergency medevac. 09:40.668 --> 09:43.560 The hospital team today, as it has in the past, 09:43.560 --> 09:46.064 was ready to meet the needs of each patient 09:46.064 --> 09:48.902 with attention to quality and safety. 09:48.902 --> 09:50.630 - Now let me talk about how the US Naval Hospital 09:50.630 --> 09:53.734 of Guantanamo Bay is also having an impact 09:53.734 --> 09:56.087 across Navy medicine for the future 09:56.087 --> 09:57.710 and the very important matter 09:57.710 --> 10:00.276 of the safe delivery of babies. 10:00.276 --> 10:02.184 Each year, about 25 babies are born here 10:02.184 --> 10:03.991 at the Naval Hospital. 10:03.991 --> 10:06.318 That's not a great number compared to other hospitals, 10:06.318 --> 10:09.996 but each of those babies is precious to their families. 10:09.996 --> 10:12.490 So safe deliveries in a remote area like this 10:12.490 --> 10:14.407 is our highest concern. 10:16.141 --> 10:19.974 In April of 2016 we invited all four perinatal 10:20.376 --> 10:24.311 specialty leaders here and they arrived as a team. 10:24.311 --> 10:26.359 We were able to engage in productive cross specialty 10:26.359 --> 10:29.482 discussions that would have been impossible to capture 10:29.482 --> 10:32.315 via email or video teleconference. 10:32.931 --> 10:36.129 The discussions ranged from provider, nursing staffing, 10:36.129 --> 10:39.084 and competencies, telemedicine capabilities, 10:39.084 --> 10:43.251 simulation training, medevacs, and pharmacy services. 10:43.780 --> 10:46.159 Face to face conversation and eyes on 10:46.159 --> 10:48.848 by the specialty leaders of this unique baby delivery 10:48.848 --> 10:51.720 environment elicited questions across specialties 10:51.720 --> 10:53.535 that could be immediately answered 10:53.535 --> 10:56.025 by all involved parties and offer to venue 10:56.025 --> 10:59.459 for discussions on the way forward in real time. 10:59.459 --> 11:01.629 At the conclusion of their visit, 11:01.629 --> 11:04.906 the specialty leaders expressed how refreshing 11:04.906 --> 11:09.038 and energizing it was to be engaged in a proactive effort 11:09.038 --> 11:11.656 and not being called on to fact find 11:11.656 --> 11:13.739 after an adverse outcome. 11:13.994 --> 11:16.361 The specialty leaders also indicated 11:16.361 --> 11:18.847 that potential clinical practice in manning solutions 11:18.847 --> 11:22.188 in Guantanamo Bay will serve as a framework 11:22.188 --> 11:26.355 for future visits at other remote overseas facilities. 11:27.063 --> 11:30.229 They saw the visit as especially fortuitous 11:30.229 --> 11:32.978 as they are helping to coordinate preparations 11:32.978 --> 11:36.442 for the opening of a birthing center in Iwakuni, Japan 11:36.442 --> 11:40.586 where many of the same challenges will likely be faced. 11:40.586 --> 11:43.586 -
